Dear Parent/Guardian:

Greetings from the Infirmary! Verde Valley School is committed to providing your child with the best possible care while at VVS. It is with this goal in mind that the school requests that the following requirements be met. Enclosed you will find several time dated medical forms that need your careful attention. These forms must be completed and returned to the VVS Infirmary by Friday, June 25, 2010. We have provided a self-addressed envelope for your convenience. I am looking forward to meeting all of the new students and seeing all of the returning students on campus in August.  It will be another wonderful school year.

First: the VVS PHYSICAL EXAM is required and needs to be on file in the Infirmary. Please call your child’s physician now to schedule an appointment to insure a timely check-up. The physical exam needs to have been done within three months before admission. Please inform your physician that the annual test for Tuberculosis is required for attendance at VVS. If your child has had a positive TB skin test, or if they have received the BCG vaccine, a negative chest x-ray for clearance is indicated and required.

If your child will be participating in a competitive sport within the Arizona Interscholastic Association (AIA), such as soccer, basketball, tennis, or golf, a separate (AIA) physical exam will be required. This form is enclosed within this packet, and must be filled out, signed and dated by a medical professional and returned, along with the other forms. This form does not take the place of the ‘Physical Exam’ form. 

In accordance with the Arizona State Department of Health, a complete Immunization Record is required and must include both the month and the year of the immunization series. This record must be completed and signed by a medical professional. If you wish your child to be exempt from immunization due to medical, personal beliefs, or laboratory reasons, then an ‘Exemption from Immunization’ form must be completed as directed. A copy of the 2010-2011 AZ School Immunization Requirements is included in this packet for you to present to your health care provider for assistance in compliance of the required immunizations.

Second: the MEDICAL, DENTAL, AND PSYCHIATRIC TREATMENT CONSENT FORM is required. Please note that your child cannot participate in any VVS sponsored activity, including, but not limited to classes and sports, until both the physical exam and consent forms are completed and reviewed by the staff in the Infirmary.

Third: the PARENT AND STUDENT AGREEMENT FOR MEDICATIONS FORM must be filled out, signed, and dated, by both the student and the parent/guardian. Your child will need a separate form for each medication he/she takes, so make copies of this form as necessary.  

Fourth: PROOF OF HEALTH INSURANCE. Every student must have proof of health coverage by either a U.S. based insurance company, or a travel health insurance policy. Foreign insurance will not be accepted; as it cannot be billed by Doctors’ offices in the U.S. Proof of health insurance coverage consists of a legible copy of the insurance card, including front and back sides. Verde Valley School will purchase a policy (which will be charged to the student account), for any international student who cannot show proof of acceptable coverage.

 As mandated by the Arizona State Department of Health. VVS will provide a hearing screening test, free of charge, to all ninth grade students, and/or any student who is thought to be having difficulties with hearing. This may be requested by a parent/guardian, or faculty member.   

In addition to the enclosed packet, you may also access these forms via the VVS website. Once again, we ask that you pay close attention to the Friday, June 25, deadline for submission of these forms. Please understand that if any one of these requirements are not met prior to your child’s arrival to campus, they will not be permitted on campus.
