GUIDE TO ENROLLMENT FORMS

(For enrolled students only. Applicants need not complete until after you have received your letter of acceptance.)

This page is designed to assist you with the completion and return of the enrollment forms you'll need to complete in order for your child to attend the 2009-2010 school year at Verde Valley School.

The following forms are REQUIRED before your student arrives and attends VVS: 

Student Information
Handbook and Activities Release
Health Record and Physical Examination including Required Immunizations

or

Immunization Waiver
Treatment Consent Form, including proof of US-Based Health Insurance

For the safety and consideration of all in the VVS community, students arriving at VVS without submission of the above forms will not be allowed on campus, nor will the student be allowed to attend classes.
