As you may know, there are certain health requirements mandated by the State of Arizona for school attendance. In addition to the Arizona requirements, Verde Valley School also has requirements that must be fulfilled prior to your son or daughter arriving on campus. I would like to share those requirements with you.

Every student must have a completed physical prior to arriving on campus. Please provide your physician with the  Arizona Immunization Chart in order to have all vaccinations up-to-date. Note that there are new Arizona State requirements for vaccinations for the 2008-2009 school year. No student will be allowed on campus until all immunizations have been completed, as per Arizona State Dept. of Health.

Periodically, we have families that request an ‘exemption’ from immunizations. If an exemption is requested, the appropriate form will need to be filled out and signed, according to the directions specified on said form. You may contact the Infirmary for a form, or download it from the Arizona Dept of Health website: www.azdhs.gov/phs/immun/

As our student body is comprised of students from around the world, Verde Valley School has determined that it is necessary for all students to show proof of a negative TB test or a negative chest x-ray. This is for your child’s benefit, as I am sure you realize. Therefore, proof of a negative TB test or negative x-ray is required before your child can arrive on campus.

While not required, we strongly recommend that your son/daughter have a vision screening test, prior to arriving on campus. Verde Valley School will provide a free hearing screening for all students.
