VVS Health Center

VVS is committed to providing your child with the best possible care while at VVS. It is with this goal in mind that the school requests that the following requirements be met, please understand that if any one of these requirements are not met prior to your child’s arrival to campus they will not be accepted for re-enrollment or as a new student:

Every student must arrive to school with proof of  health coverage by either:

•    a U.S.-based insurance company

•    or a travel insurance policy

Foreign insurance will not be accepted, as it cannot be billed by doctors’ offices in the U.S.

Proof of health insurance coverage consists of a legible copy of the insurance card, front and back sides.

If your child needs a travel insurance policy, please contact the school regarding the travel insurance policy offered through VVS. The policy can be purchased through our business office. You can, of course, purchase travel insurance on your own, but it must be travel health insurance.



Every student must arrive with a physical exam completed by a licensed health care provider:


•    the physical needs to have been done within three months before admission

•    it needs to include a complete immunization history (please note: for returning students, please include their immunization history, even if it has been sent the previous year, send it again) If you wish your child to be exempt from immunization due to religious beliefs, or health practices, then an “Exemption from Immunization” form must be completed, (one has been included in the packet).

•    Arizona state law requires that all students have proof of TB clearance, so the physical must include a PPD skin test, or if your child has tested positive in the past, a chest x ray, or if they have received the BCG vaccine, a chest x-ray for clearance is indicated. (If you have any questions regarding this, bring this piece of paper to the doctor, they will know what to do).

•    Arizona state law requires that every student age 16 or younger requires a vision and hearing screening performed by a licensed screener upon the start of each school year. Please include that screening information with the physical if that applies to your child.

-every student must arrive with a signed treatment consent form:

•    this allows the school to provide any services to your child during their stay.
•   
•    please make sure that the billing address information on this form is printed and legible.

-every student must arrive with a medication agreement:

•    this form needs to be filled out and signed by both the student and parent/guardian.

These are the requirements from a health perspective for your child to be enrolled in the fall, again, if any one of these requirements are not met, your child will not be allowed to enroll. Increased enrollment has made this an absolute necessity.

If you require your child to see a provider in the area, our health office staff will arrange the appointment and transportation.
